Household Income Data Collection

For our the 2024-2025 school year the United States Department of Agriculture (USDA) has allowed school districts to continue to feed all children breakfast and lunch meals at no cost. Since meals are free for all our students, families will not need to fill out the typical meal application to receive those meals. All student’s benefit by having access to nutritious breakfast and lunch meals. Under the Local Control Funding Formula (LCFF), a portion of state-funding is determined based on the demographics of a districts student population. We typically use the USDA meal applications to collect this data for the state. Since we will not have meal applications to acquire these numbers we need to collect this information by having families complete the Education Benefit Form. By taking the time to fill out this form you are taking part in a process that provides thousands of dollars in funds to the school your student attends. Our goal is to ensure that Gold Oak Union School District’s students receive the maximum amount of funds that we are eligible to receive by encouraging ALL families to complete this form. To receive the maximum amount we are eligible to receive, we must have 100% of students/families complete the form below.  Please return to the school office.

Nondiscrimination Statement for Nutrition Programs

NONDISCRIMINATION STATEMENT FOR NUTRITION PROGRAMS

The following statement shall be included, in a prominent location, on all forms available to the public regarding the availability of the district's child nutrition programs:

"In accordance with Federal civil rights law and U.S. Department of Agriculture (USDA) civil rights regulations and policies, this institution is prohibited from discriminating on the basis of race, color, national origin, sex (including gender identity and sexual orientation), disability, age, or reprisal or retaliation for prior civil rights activity.

Program information may be made available in languages other than English. Persons with disabilities who require alternative means of communication to obtain program information (e.g., Braille, large print, audiotape, American Sign Language, etc.), should contact the responsible state or local agency that administers the program or USDA's TARGET Center at (202) 720-2600 (voice and TTY) or contact USDA through the Federal Relay Service at (800) 877-8339.

To file a program discrimination complaint, a Complainant should complete a Form AD-3027, USDA Program Discrimination Complaint Form, which can be obtained online at:

The letter must contain the complainant's name, address, telephone number, and a written description of the alleged discriminatory action in sufficient detail to inform the Assistant Secretary for Civil Rights (ASCR) about the nature and date of an alleged civil rights violation. The completed AD-3027 form or letter must be submitted to USDA by:

Unpaid and Delinquent Meal Charges—Board Policy/AR 3551

Students and their parents/guardians shall be notified whenever their account has a low or negative balance.  Whenever a student’s account has an unpaid balance of $10 or more, parents/guardians shall be notified in writing that full payment is due within seven school days from the date of the notice.

In cases of repeated nonpayment by a student, the Superintendent or designee may contact parents/guardians to discuss the reason for the nonpayment.  The Superintendent or designee may evaluate individual circumstances to determine if the student’s parents/guardians need assistance completing an application for free or reduced-price meals or need a referral to social services.

The Superintendent or designee may enter into a repayment plan with a student’s parents/guardians for payment of the student’s unpaid meal charge balance over a period of time.  As necessary, the repayment plan may allow the unrecovered or delinquent debt to carry over into the next fiscal year.
